If you have been referred to Dr Hazem Fallouh by your GP or chest physician, or if you are self-referring yourself following a recommendation from friends or a previous patient we have treated, complete the form below and we will arrange a private consultation at your convenience.
At this consultation Dr Hazem Fallouh will be able to discuss your case in detail with the relevant information at hand. This would be collated prior to the consultation. Your specific needs can then be discussed from both a surgical and administrative point of view.
